By signing this form below, I, [9] acknowledge responsibility of ensuring the electronic devices above will be kept in working order and also agree to each of the following terms: Equipment will be used for Whiz Kidz Preschool purposes only. I will keep the devices clean and preserve them in excellent condition. I will keep devices in the assigned classrooms. I will place equipment with docs and charge each closing day. I will let the Center Director know when equipment needs IT support. I will not download or upload any inappropriate, illegal or obscene materials on company devices. I will also no longer use my personal phone to manage and operate the classrooms. Starting January 15th, 2023, use of all personal cell phones will no longer be permitted, unless the employee is clocked out for the day and/or clocked out for a break on their personal time.
